If you are wondering whether genetic counseling might be of value to you, you are invited to complete this family history survey.
Please read about cancer genetic counseling and complete the questionnaire to prepare for your appointment. The questionnaire can be completed electronically and e-mailed to Vanessa Marcell (Vanessa_Marcell@bshsi.org) in advance of your appointment. Otherwise, please print the questionnaire and bring it with you and any other relevant medical records to your appointment.
