Your medical record contains important information about your health evaluation/assessments, diagnosis and treatments you received at WMCHealth. It is a valuable communication tool between WMCHealth and your other healthcare providers and supports any continuing care you may need or desire. Use the below links to access your medical records.

Patient Portal

myCare, the WMCHealth patient portal, is a secure online website that gives you convenient 24-hour access to your personal health information. You can sign up for myCare during the registration process in the hospital, at your doctor’s office or here:

Requesting Copies of Medical Records

You may request a copy of your Medical Records at any time. To make the request, click here.

Records can be provided upon request via CD or flash drive, or sent to you electronically by email. Please specify your preferred format on the authorization form. Requests are processed within five to 10 days of receipt. For records that are mailed, please allow time for delivery. You or a representative designated by you may pick up the medical records and must show a valid ID at time of pick-up. 

New York State Public Health Law allows WMCHealth to charge a reasonable fee to recover the costs of copying, mailing and supplies used to fulfill your request. Patients will receive a pre-bill or a payment notice with their records (a significant discount is applied if electronic copies are requested). However, there is no fee if the record is being released to a doctor or healthcare provider.

Linking Your Medical Records

To give you the safest, best care, your health care provider usually needs a lot of information: your medical history, allergies, prescriptions, specialist visits, lab tests and more. There are two programs at the state and national level that allow you to link your information in one place.

HIXNY

The Healthcare Information Xchange of New York (HIXNY pronounced HIX-KNEE) is part of a New York statewide network that you can voluntarily use to help collect the medical records you have in different places in New York State where you get health care, and make them available electronically. The benefits of having accurate, up-to-date information include fewer repeated tests, reduced risk of mistakes, easier second opinions and less chance of drug interactions.

CommonWell

CommonWell provides our patients the opportunity to allow your participating health care practitioners access to your health information nationwide, regardless of where care occurs.

You can choose whether to allow Westchester Medical Center, Advanced Physician Services, P.C. and MidHudson Regional Medical Center to obtain access to your medical records through HIXNY and CommonWell. Your choices will not affect your ability to get medical care or health insurance coverage.
