The Case Management Department at Good Samaritan Hospital consists of a team of nurses and social workers. The case managers screen all patients admitted to the hospital to assess the patient’s future health care needs once he/she leaves the hospital.

Case managers work closely with patients, their families, and a multidisciplinary team (which includes nurses, physicians, dietitians, and rehabilitation therapists) to establish a customized discharge plan to ensure a smooth transition from one level of healthcare to another. This may include sub-acute/acute placement, home care, or outpatient services.

The case managers conduct utilization reviews to ensure that patients are in the appropriate level of care. They work with insurance carriers to determine coverage and reimbursement under the patient’s contract.

The Case Management Department is a member of the hospital-wide Quality Assurance Committee.
