Bon Secours Medical Group Community Outreach Provider Program

Keeping You Healthy at Home

The Community Outreach Provider Program (COPP) is for Rockland County residents in collaboration with Rockland Paramedic Services (RPS) which allows for a paramedic to provide health and wellness care in your home.

This service is made available through the Bon Secours Medical Group, a member of the Westchester Medical Center Health Network.

COPP is managed by a coordinator that is an advanced practice clinician, who will assess your healthcare needs and is available to all enrollees.

This program allows paramedics to function outside their traditional emergency response roles and utilize their skills to provide care in your home.

Program Services

  • Evaluation and treatment of non-life-threatening conditions
  • Education on management of medical conditions
  • Referral to community resources
  • Home evaluation of medications and instructions post-hospital or rehab discharge
  • Wound checks
  • Lab work
  • EKGs and chest x-rays

The coordinator will work with your doctor regarding any treatment provided by the paramedic or changes in your condition.

Who is Eligible?

Residents of Rockland County with healthcare needs or who require follow-up care in the home can enroll.

What is the Cost?

This program is covered by most insurances, including Medicaid.
